Hello again!

With our second issue, we bring you a few more updates that have happened in the Qbank.

In this newsletter, the focus is on MCQs that have been revised based on non-COVID related recent updates.

1. 3 Drug Regimen for HIV Post Exposure Prophylaxis

The NACO October 2018 guidelines had recommended a 2 drug regimen for the PEP prophylaxis of HIV. 

The recent MOHFW January 2020 guidelines on the management of PEP in HIV recommends the replacement of 2 drug regimen with 3 drug regimen referring to WHO December 2018 guidelines on PEP management protocol in HIV. This shift towards recommending a three-drug regimen for
everyone was based on the availability of less toxic and better-tolerated medications.

Recommended PEP regimen:

Drugs combinationRecommendation for PEPDuration
Tenofovir (TDF) 300 mg +
Lamivudine(3TC) 300 mg +
Dolutegravir (DTG) 50mg
Once-daily4 weeks
DTG is recommended as the preferred third drug for HIV post-exposure prophylaxis.

When available, Atazanavir, Darunavir, Lopinavir, and Raltegravir may be considered as alternative third drug options for post-exposure prophylaxis.

2. MTP AMENDMENT BILL 2020

Medical Termination of Pregnancy (MTP) Amendment Bill 2020 states that for MTP to be done, the opinion of one registered medical practitioner is required for pregnancy up to 20 weeks of gestation.
The opinion of two registered medical practitioners is required when the length of the pregnancy exceeds 20 weeks but does not exceed 24 weeks.

The upper gestation limit for MTP which was twenty as per MTP act, 1971, has been increased to twenty-four weeks in this bill.

Note: The bill has been passed by the Lok Sabha and is tabled at the Rajya Sabha for discussion.

3. BLINDNESS DEFINITION (WHO):

WHO has recently revised the definition for blindness.

The term “visual impairment” now includes moderate and severe visual impairment as well as blindness.

Moderate visual impairment is defined as a presenting visual acuity in the range from worse than 6/18 to 6/60

Severe visual impairment is defined as a presenting visual acuity of worse than 6/60 and equal to or better than 3/60.

As per the new definition, blindness is defined as a presenting visual acuity of worse than 3/60 or a corresponding visual field loss to less than 10° in the better eye.
